Ankylosing spondylitis is often missed for years. A thoughtful evaluation can bring clarity – and a treatment plan that actually addresses what’s driving the pain.
Ankylosing spondylitis is an inflammatory autoimmune condition that primarily affects the spine and the sacroiliac joints, where the spine meets the pelvis. The hallmark is inflammation that creates stiffness and back pain with a distinct pattern – often worse with rest and better with movement.
Patients with Ankylosing Spondylitis may experience:
Low back or buttock pain that improves with movement and worsens with rest
Significant morning stiffness lasting more than 30 minutes
Pain that wakes you in the second half of the night
Onset of symptoms before age 45
Fatigue and reduced flexibility over time
Sometimes peripheral joint pain, eye inflammation, or GI symptoms
Inflammatory back pain can look a lot like mechanical back pain on the surface. Imaging changes can take years to appear, and not all patients carry the HLA-B27 marker. A careful clinical evaluation is often what brings the picture into focus.
The pattern of back pain - when it's worst, what helps, what doesn't - often tells us more than any single test.
Targeted imaging, HLA-B27 and inflammatory markers when appropriate, and a careful exam of the spine and peripheral joints.
Eye inflammation, GI symptoms, peripheral joints, and family history all factor in. AS rarely lives in isolation.
